Premiums include £8.00 policy fee, insurance premium tax at 6% and are subject to a satisfactory proposal form proposal form.

How to arrange cover

1. Complete and sign the proposal form.
2. Return the form with a cheque payable to John Russell Insurance Services Limited. Alternatively enter your credit card details on the form.
3. Cover will be effected upon receipt of the form (or later date if shown on proposal).
